Notice: Weapons Detection Screening
For the safety and security of our patients, visitors and team members, Signature Health has implemented a weapons detection system. Individuals entering the health center will be subject to screening. We appreciate your cooperation and understanding as we work to maintain a safe environment for everyone. Items that some individuals may carry regularly, such as pocket knives, box cutters, and pepper spray, should be left in the car or at home (and will need to be left with the security officer if found and a car isn't available for safekeeping). Firearms are not allowed to remain in the building under any circumstances. If you have any questions, please call the office and ask to speak with the RN Site Manager.

Mental Health Clinic


Counseling and psychiatry services are available for both children and adults with mental health concerns, making it easier to find therapy near you and seek ongoing support in the Maple Heights area. Our mental health clinic includes specialized therapy for trauma (EMDR), and case management services.

Addiction Recovery


Recovery from opioid, drug, alcohol, or gambling addiction starts here. Each patient receives comprehensive care, which may include an addiction counselor, Medication-Assisted Treatment (MAT), our Intensive Outpatient Program (IOP) services, and more.

Primary Care


Our primary care physicians and clinicians provide services to patients of all ages including physicals, treatment for common illnesses and injuries, vaccinations, screenings, prevention or management of chronic conditions, infectious diseases, and sexual and reproductive health.

Laboratory


On-site blood draws and lab services make screenings simple and convenient for our patients at our Maple Heights health center.

Thursday | 9:30am-6:00pm

The last urine screening will be 15 minutes before the lab closing time listed, and the last blood draw will be done 30 minutes before the lab closing time listed.

How can I get help as a new patient?

If you have 60 minutes available now, you can speak with a counselor over video for an assessment (Mon-Fri, 9am-4pm).

Do you offer transportation?

Yes, to qualifying patients. If you are regularly unable to make it to your appointments because of lack of transportation, please let us know.

It is staffed Monday-Friday, 7 a.m.-6 p.m. An RTA staffer will answer your questions. Be prepared to provide them with the address of your current location and the address of your destination.

How do I pay for care?

Signature Health accepts:

  • Medicaid
  • Traditional Medicare
  • Some Medicare Advantage plans

For patients without insurance, we offer a sliding fee scale program for those who qualify. This means payment is based on your income and household size. We can also help get new patients set up with Medicaid. See a list of accepted insurances.
