Signature Health is not considered in-network for most commercial insurance plans. If you have private insurance, it is recommended that you contact your insurance provider for a list of in-network clinicians.

Sliding Fee Scale Program

Based on household income and size, patients who qualify for the Sliding Fee Scale Program pay as little as $0, $5, $10 or $20 for services. See the tiers.

Self Pay

For patients who do not have an insurance that is accepted by Signature Health and do not qualify for any of our financial assistance programs, self-pay is an option. Payment is due prior to the time of service.
