Signature Health’s team in Lakewood cares for both children and adults. We offer mental health, addiction recovery, infectious disease, primary care and pharmacy services—through in-person and telehealth appointments. Several different counseling groups are available to meet our patients’ needs, as well as free, walk-in screening for HIV and hepatitis C.
To establish mental health or addiction recovery care, patients complete a 90-minute assessment to determine appropriate services. These appointments are typically scheduled in advance, or we offer some walk-in slots Monday through Friday. To schedule a new patient assessment, you can call the office, schedule online, or live chat a patient services team member during business hours.

Wed: 8am - 1pm (telehealth only; except no primary care available on the 3rd Wed. of the month)
