School-Based Services
Signature Health works with local school districts to provide behavioral health care for students in elementary through high school. We can build a program to meet your school and students' needs.
How It Works
Referral & Consent
Students are typically referred for services through their school. A parent or guardian must complete consent forms before services begin.
Assessment & Services
A clinician may complete an initial diagnostic assessment to better understand the student’s needs. Services are then provided during the school day.
Privacy & Communication
Information is only shared with between Signature Health and school staff when a parent or guardian signs a Release of Information, allowing coordination while maintaining confidentiality.
Payment & Insurance
Signature Health accepts Medicaid and some Medicare plans. We will review coverage and any potential costs with you before services begin.
Stay Connected
Engaging in care with a Signature Health school-based services means a student gets registered as a Signature Health patient. Parents and guardians can activate their student in MyChart to access appointment summaries, provider notes and more.
School-Based Services Available
Diagnostic Assessments
A diagnostic assessment is a clinical evaluation completed by a licensed professional to better understand a student’s needs. This includes gathering information about current concerns, emotional well-being, and overall functioning.
Based on the assessment, the clinician will make recommendations and begin developing an individualized treatment plan. A diagnostic assessment is required before beginning individual or group counseling.
Risk & Safety Assessments
Risk and safety assessments are provided during urgent or crisis situations if a student may at risk of harm to themselves or others.
Individual Counseling
Students can participate in one-on-one counseling sessions with a licensed professional to work toward personal goals and improve overall well-being.
Our team supports students experiencing concerns such as trauma, anxiety, depression, and behavioral challenges that may impact school performance. We encourage family involvement and work closely with caregivers to support each student’s progress.
Group Counseling
Group counseling sessions allow students to work on shared goals in a supportive, structured setting with peers.
Case Management
Student Assistance Programs (SAP)
Signature Health providers may support schools through Student Assistance Program (SAP) services, including:
- Participation in parent or IEP meetings
- Short-term skill-building group sessions
- Classroom-based presentations
A diagnostic assessment is not required for these services.

Family Counseling
Family counseling helps families navigate challenges together, work through conflict, strengthen bonds, and move forward with greater understanding and resilience.
Children's Counseling
Through compassionate, age-appropriate therapy, we help kids and teens break down big feelings into manageable steps, build coping skills, and move toward healing and growth.
Parent-Child Interaction Therapy
Parent-Child Interaction Therapy (PCIT) is offered in Ashtabula and Willoughby. PCIT empowers parents and caregivers with effective tools to manage behavior while fostering a more positive and nurturing connection with their child.
Community-Based Family Therapy
Community-Based Family Therapy (CBFT) provides intensive, family-centered support to children, adolescents, and their caregivers within the Lake County community and home environment.
Sexual Behavior Services
Our sexual behavior services include age-appropriate programs that are designed to address problematic or abusive sexual behaviors while promoting safe, respectful, and healthy sexual expression.
