Eligibility & Referrals
Eligibility
C.H. Everett Clinic serves adults age 18 and older who:

Need step-down care following a psychiatric inpatient or crisis stabilization stay

Need step-up care and a higher level of support than outpatient treatment can provide
- Would benefit from a structured residential environment focused on stabilization and recovery
Referrals
Referrals must be submitted by a qualified behavioral health professional and are reviewed by the C.H. Everett Clinic Manager. Download and complete Signature Health's Residential Admission Form.
