What is Case Management?

Case Management is a comprehensive process of planning, coordinating care, and advocating for a patient to make sure their health needs are met. Through communicating available resources and promoting safety and quality of care, Case Managers can help patients live their best lives.

How to be seen by a Case Manager?

If you need help with mental health or substance use, Signature Health will put you on the right path by completing an initial assessment. During this session, we will discuss your needs and past medical and mental health history. Our intake specialist will also discuss our available services and recommend next steps, including assigning you to a Case Manager if needed.

Frequently Asked Questions

How can I get help as a new patient?

The first step is to start with an assessment. If you have 60 minutes available now, you can speak with a counselor over video for an assessment (Mon-Fri, 9am-4pm).

To schedule an appointment, call, chat or book your spot online.

Do you offer transportation?

Yes, to qualifying patients. Each of our locations is also located near a main bus route for patient convenience. If you are regularly unable to make it to your appointments because of lack of transportation, please let us know.

How do I pay for care?

Signature Health accepts:

  • Medicaid
  • Traditional Medicare
  • Some Medicare Advantage plans

For patients without insurance, we offer a sliding fee scale program for those who qualify. This means payment is based on your income and household size. We can also help get new patients set up with Medicaid. See a list of accepted insurances.
