If an appointment does not appear within the desired time frame or location, please call 440-578-8211 to speak with a Signature Health team member about other available options. 

After completion of the form, the appointment is scheduled. An email confirmation and reminder texts will be sent to the patient.

If the patient needs an interpreter at their appointment, please note this in the 'comments' field along with the medical reason for visit (e.g. pneumonia, needs Spanish language interpreter).   

If the patient requires transportation to their upcoming appointment, please note this in the 'comments' field along with the medical reason for visit (e.g. pneumonia, needs transportation).

Because a Signature Health Patient Navigator will need to reach out to the patient to explore options, please avoid scheduling a next-day appointment for patients who need transportation and please encourage the patient to respond to our outreach. 

We accept Medicaid, traditional Medicare, some Medicare advantage plans, and offer a sliding fee scale for uninsured or underinsured individuals who qualify. See list. 

If we cannot verify insurance or the patient needs to connect with a financial assistance rep, we will reach out prior to the appointment, so please encourage the patient to answer/respond to our outreach.  

Signature Health also offers online scheduling for behavioral health assessments, for patients who would benefit from ongoing counseling, psychiatry or addiction medicine services. Schedule now. 

If the patient will remain in the hospital past today and you wish to coordinate for a counselor liaison to conduct a behavioral health assessment in-person at the bedside (M-F), email Cuyahoga-BHliaison@shinc.org to schedule.
