Private, Commercial, or Employer Insurance
Signature Health is not considered in-network for most commercial insurance plans. If you have private insurance, contact your insurance provider for a list of in-network clinicians. If you’d like to discuss self-paying for care, or see if you qualify for any financial assistance, schedule a visit with a team member. Please bring to the appointment:
- Proof of any household income (e.g., pay stubs, bank statement, etc.)
- Proof of where you live (e.g., cell phone or utility bill with an address, bank statement, etc.)
