Financial Help Appointment

If you don't have insurance, the first step is to meet with a team member to see if you qualify for help to pay for care.

For this appointment, please have information ready about your household income (e.g., pay stubs, bank statement, etc.) and information showing where you live (e.g., cell phone or utility bill, bank statement, etc.)
