If you don't have insurance, the first step is to meet with a Signature Health team member to see if you qualify for help to pay for care. Please bring to this appointment:

  • Proof of any household income (e.g., pay stubs, bank statement, etc.)
  • Proof of where you live (e.g., cell phone or utility bill with an address, bank statement, etc.)
