Community-Based Family Therapy (CBFT)

Signature Health's CBFT program is designed to provide intensive, family-centered support to children, adolescents and their caregivers within the Lake County community and home environment.

About CBFT

The program’s philosophy is rooted in the Intensive Home-Based Therapy model, emphasizing: 

  • Strengthening family functioning and communication 
  • Keeping youth safely in their homes and communities 
  • Building practical skills that support long-term change 
  • Coordinating care across natural supports and community systems

Services are flexible, individualized, and responsive to each family’s unique needs, with an emphasis on meeting families where they are, both literally and figuratively.

Referral Eligibility

  • Patients appropriate for CBFT may require increased support, engagement, and coordination from a counselor/case manager treatment team
  • Referral sources are encouraged to provide relevant clinical, behavioral, and family system information to support determination of program appropriateness
  • Admission into the program will be based on clinical appropriateness, service capacity, and the ability to meet the identified needs of the family
  • Currently the CBFT Program is only available to Lake County patients with team members based in our Painesville office.

Parent-Child Interaction Therapy (PCIT) is offered in Ashtabula and Willoughby. PCIT empowers parents and caregivers with effective tools to manage behavior while fostering a more positive and nurturing connection with their child.
