Signature Health Awarded $10,000 Grant from the United Way of Lake County to Support Mental Health Care at Home for Older Adults

Kirtland, OH—Signature Health is pleased to announce a generous $10,000 grant from the United Way of Lake County, which will bolster the organization’s growing Care at Home program.

“Caring for the behavioral health needs of older adults with skill and compassion is vital for this rapidly rising population,” says Adrienne Green, Director, Geriatric Services, Signature Health. “Signature Health has made a commitment to meet community needs in this area, and we are thankful for the United Way’s support to help us serve vulnerable Lake Countians in the comfort of where they call home.”

"This grant to Signature Health’s Care at Home program is a testament to our commitment to supporting comprehensive, compassionate care for older adults in Lake County. By bringing essential mental health services directly to their homes, we are not only improving access to care but also ensuring that our seniors receive the dignified and respectful support they deserve. We are proud to partner with Signature Health in this critical initiative," said United Way of Lake County President and CEO Dione DeMitro.

The Care at Home program delivers high-quality psychiatry, counseling and case management services directly to the homes of older adults—whether they reside in a private residence, assisted living facility, or nursing home. The combination of experienced clinicians who understand the unique needs of older adults, patient-centered care with personalized care plans, and 24/7 access to the clinical team brings peace of mind to families coping with serious mental health conditions.

The grant helps to fund the role of a geriatric care liaison to provide assistance to older adults and their caregivers and support the continued development, expansion and success of the Care at Home program.

"[The Care at Home Program] has made such a difference for me...My case manager has been with me every step of the way. When I see the nurse practitioner, it's immensely helpful. I wouldn't be able to do it with telehealth. I only have an old-fashioned phone. If I didn't have these people, I don't know where I'd be," shared a Care at Home patient.

As people age, they may experience changes in their mental health that can impact their quality of life. The importance of early recognition and treatment for behavioral and mental health issues in older adults—including dementia, insomnia, and mood disorder—is crucial, as early behavioral treatment can reverse or slow disease progression.

“While there are a wide variety of social services for older adults in Lake County, there are only limited resources for aging-specific mental health care,” added Green. “Signature Health is proud to be among the very few non-profit and community health centers in the county bringing specialized geriatric services to facilities, providing psychiatric care, and delivering these services in individuals’ homes.”

For more information about the Care at Home program, contact Adrienne Green at 440-578-8200 ext. 10171.

Signature Health to Reopen Cleveland Residential Treatment Facility to Care for Individuals with Serious Mental Illness and Substance Use Disorder

ORCA House to Expand Services at New Location; Supported by ADAMHS Board of Cuyahoga County & OhioMHAS American Rescue Plan Act Funding

April 18, 2024 — Signature Health announces the reopening of ORCA House in Northeast Ohio as a pivotal enhancement to the region's mental health and addiction services. The facility, now housed in a newly renovated, 24,000-square-foot space at the intersection of Prospect and E. 30th in Cleveland, features 36 beds and expects to begin accepting referrals the week of April 29. This expansion is made possible through funding support from Ohio Department of Mental Health & Addiction Services (OhioMHAS) American Rescue Plan Act (ARPA) funds and the Alcohol, Drug Addiction & Mental Health Services (ADAMHS) Board of Cuyahoga County.

With 16 beds dedicated to adults with serious mental illness, the mental health treatment floor will offer step-down care for patients discharged from inpatient psychiatric hospitalization, as well as step-up care for individuals not needing hospitalization but requiring monitored residential treatment. Services include behavioral health assessments, case management, peer support, psychiatry, nursing, and individual and group counseling with an emphasis on Mental Health Day Treatment groups.

“We are well-positioned at ORCA House to welcome residents into a warm, structured environment that feels like home. Our approach centers on stabilizing symptoms, promoting individual growth and development, and supporting successful transition back into the community,” said Jonathan Lee, CE0 of Signature Health. “Strong collaboration with our funders and other partners has made this new community resource possible, to offer hope and the right level of compassionate care to individuals facing significant mental health and addiction challenges.”

Scott Osiecki, CEO of the ADAMHS Board of Cuyahoga County said, “I am grateful that Signature Health stepped-up to increase access to transitional residential treatment as part of our safety net that will assist in combating suicide and preventing re-hospitalizations. I am also thankful to OhioMHAS for providing the resources to establish this step-down/step-up facility in our community.”

The facility also anticipates the future opening of a 20-bed unit aimed at treating individuals with opioid use disorder or alcohol use disorder, with an emphasis on intensive group therapy and medication-assisted treatment (MAT). This unit also will provide ongoing medical care for residents discharged from the hospital who have co-occurring substance use disorder and endocarditis, which is a potentially fatal inflammation of the heart valves.

“Creating access for ORCA residents to receive high-quality treatment for both opioid use disorder and endocarditis in the same care facility represents a pioneering approach to integrated care that was identified by our local hospital systems and payers as a need in our community,” said Lee. “We have been fortunate to collaborate with CareSource on a care and reimbursement model aimed at reducing future hospitalizations and total cost of care.”

"Access to evidenced based treatment for Opioid Use Disorder, including medication assisted treatment, while receiving intravenous antibiotics for infective endocarditis is essential,” said Dr. Mark Reynolds, CareSource Enterprise Medical Director, Care Management. “CareSource is excited to work with Signature Health in bringing this innovative initiative to Northeast Ohio."

With deep roots in Cleveland since 1942, ORCA House is one of the oldest African American-founded centers for substance use disorder in the country. It became part of Signature Health in 2018, and ORCA’s former location on E. 89th Street closed in 2020 due to the COVID-19 pandemic and need for renovation.

In addition to over $1 million in ARPA and ADAMHS Board funding, additional support for ORCA House was provided by the Cleveland Foundation and two of its affiliated funds, the Higley Fund and the Thatcher Family Fund.

The average length of stay at ORCA House is expected to be 30 to 90 days, with around-the-clock coverage from nurses and residential care specialists. For more information on referring to ORCA House, visit https://www.signaturehealthinc.org/locations/residential-facilities/orca-house/

Signature Health Launches Patient Navigation Services to Support Care Coordination and Treatment Access for Northeast Ohio Individuals with Behavioral Health Needs

More than $500K in funding from Anthem Blue Cross and Blue Shield Foundation and the Lake County Alcohol, Drug Addiction, and Mental Health Services (ADAMHS) Board addresses care gaps through patient navigation services.

November 2, 2022: Signature Health now offers patient navigation services to further support individuals who have taken the first step in seeking treatment for a mental health and/or a substance use disorder (SUD). The new program is supported through grant funding from two organizations.

Signature Health’s growing team of patient navigators—which also includes personnel support from AmeriCorps—outreaches to newer patients, to offer personalized support, education, care coordination, and resources to address the impact of social determinants of health. Navigators are trained to understand systemic barriers to health care and support members of historically marginalized communities, including patients of color, patients with disabilities, and low-income patients.

According to SAMHSA’s 2020 National Survey on Drug Use and Health, approximately 73.8 million Americans (more than 29%) had a mental illness and/or substance use disorder, illustrating the importance for programs like this to support patients in successfully navigating their treatment needs.

“Our agency data and experience in behavioral health care tells us that patients who reach their seventh appointment are more likely to stay engaged with treatment and see long-term, positive health outcomes,” said Jonathan Lee, CEO, Signature Health. “However, many people reach out for help but lose the courage to follow through before we see them even the first time. By including dedicated Patient Navigators as part of the care team, we’re able to provide higher-touch, concierge-like services to help our newer patients better understand the programs available to them, and drill down into any barriers that pose a risk to the individual disengaging with the treatment they need. We are deeply appreciative of the grant funding provided by Anthem Blue Cross and Blue Shield Foundation and the Lake County ADAMHS Board. This funding allows us to build out a passionate team of patient navigators and further protect against disengaged patients falling through the cracks.”

Anthem Blue Cross and Blue Shield Foundation has awarded Signature Health $350,000 over three years to fund navigation services for patients living with SUD, to support access to treatment and lifelong recovery. This grant is part of a larger Anthem Blue Cross and Blue Shield Foundation investment of up to $30 million over the next three years across the nation—with nearly $1.1 million in Ohio—to promote equity in mental health, particularly for people with SUD.

“We know a more personalized approach to improving health lies within our communities and we are grateful to work alongside Signature Health to provide meaningful solutions to achieve better health and to advance health equity,” said Dr. Benjamin Boche, Anthem Blue Cross and Blue Shield’s Medicaid Behavioral Health Medical Director in Ohio. “By acknowledging the physical, behavioral, and social drivers of health as they relate to substance use disorders, the Anthem Blue Cross and Blue Shield Foundation, along with our community partners, are positioned to provide real progress for those that may be facing challenges.”

The Lake County ADAMHS Board has awarded Signature Health $180,000, also in support of patient navigation services for behavioral health and primary care patients, with a focus on integrated care.

Kim Fraser, Lake County ADAMHS Board Executive Director said, “When we reviewed the proposal, immediately we saw the potential benefits, including ease of navigation and an increased likelihood that individuals in need will continue vital recovery services, leading to better long-term results. This is a great first step toward increasing success rates and providing better care to those in Lake County.”

Signature Health Ashtabula Invites Public Comments on Upcoming Renovation and Expansion Plans

Signature Health Ashtabula, which occupies the six-story “old Hotel Ashtabula” at 4726 Main Avenue, is designated as a historic property. The building stood vacant for several years before Signature Health leased, renovated and opened the space in 2014 to serve the community through outpatient behavioral health and primary care services. Now, Signature Health is planning new renovations to the building’s 1st, 5th and 6th floors.

“We are committed to preserving the building’s beloved charm while updating and expanding the space to better serve our patients, and meet the growing needs of the Ashtabula community,” said Jonathan Lee, CEO, Signature Health. “The iconic marble staircase, arched windows, high lobby ceilings, and mosaic tile on the first floor, and ballroom on the second floor, will all be preserved. Many locals have fond memories in this historic building, from proms or weddings when it was Hotel Ashtabula, and we’re proud to be part of the fabric of the community in this way.”

On the first floor, renovation plans include expanding the on-site pharmacy and lab areas. To allow for this, primary care services will relocate to the sixth floor.

The now-vacant fifth floor will transform into 6,000 sq. ft. of space dedicated to child and family counseling and a new service called Parent-Child Interaction Therapy (PCIT).

The now-vacant sixth floor will transform into 6,000 sq. ft. of space dedicated to primary care services, including nine exam rooms, two nurses stations, provider work space, a lab, and reception/waiting areas.

The renovation project is expected to begin in May 2022.

As part of Signature Health’s process, the public is invited to voice any concerns over this project plan. If you have any comments about the renovation’s possible effect on this historic property, please email SH-Marketing@shinc.org no later than February 21, 2022.

Signature Health Expands Free Screening Services with Walk-In Testing for Sexually Transmitted Infections (STIs/STDs)

(Mentor, OH): Thanks to a grant from the Ohio Department of Health, Signature Health is expanding its free, walk-in infectious disease screening services to include testing for four sexually transmitted infections (STIs/STDs): gonorrhea, chlamydia, syphilis, and trichomoniasis. This new offering was first made available to existing Signature Health patients in early August and will launch for community members on Monday, December 6. The STI screenings will be available at Signature Health’s Lakewood office, 14701 Detroit Ave., Lakewood, OH 44107, and Signature Health’s Willoughby office, 38882 Mentor Ave., Willoughby, OH 44094, during certain dates and times.

To aid in offering rapid, walk-in STI testing, Signature Health has acquired two binx io machines. This innovative piece of equipment tests and provides results for chlamydia and gonorrhea in approximately 30 minutes, so an individual leaves the office knowing their status and any prevention or treatment next steps. Signature Health is currently one of the only organizations in the state of Ohio to offer this type of testing. Testing for gonorrhea and chlamydia is done through urine analysis or a self-administered vaginal swab; testing for syphilis is done through blood with a finger stick; and trichomoniasis is tested through a self-administered vaginal swab. All tests are available to individuals 15 and older.

“Signature Health is a Northeast Ohio leader in offering accessible infectious disease care to our community members,” says Sarah Hoehnen, DO, Medical Director of Infectious Disease at Signature Health. “In addition to sharing a positive or negative result, our prevention counselors provide resources and can link people with treatment or prevention services, often that same day if needed.”

This STI screening program supplements the free, rapid, walk-in HIV and hepatitis C testing that Signature Health already has in place for community members and patients. Depending on the combination of screenings received, all results are typically available on the same day, often in as little as 30 minutes. The organization’s infectious disease providers offer PrEP and PEP for HIV prevention, as well as expedited treatment for anyone who tests positive for HIV, hepatitis or STIs.

Everyone should be screened for HIV and hepatitis C at least once. Everyone who has ever been sexually active should be screened for sexually transmitted infections, which are particularly common among people ages 15 to 24.

For a full calendar of STI/HIV/Hep C walk-in screening locations, dates and times, visit SignatureFreeTest.org. You also can call 440-578-8200.

HIV and Hep C Testing Resumes

Cleveland-based Signature Health, a health center providing primary medical care, infectious disease, behavioral health, and substance use disorder services is offering free HIV and hepatitis C testing at six Signature Health locations, and the LGBT Community Center of Greater Cleveland. The program began last year and was temporarily on hold due to COVID-19.
Available Monday-Friday, Signature Health will provide this free, confidential HIV and hepatitis C testing on a walk-in basis at their Lakewood, Beachwood, Maple Heights, Willoughby, Painesville, Ashtabula locations, and at the LGBT Community Center of Greater Cleveland. For the full testing schedule, please see the website. One does not have to be a patient and an appointment is not necessary. To preserve confidentiality, anyone desiring a test should simply ask for “free testing” at the check-in window. The testing is free thanks to funding provided by the Cuyahoga County Board of Health, Ohio Department of Health, and HRSA. Please note, Signature Health is following COVID-19 safety precautions, as recommended by the state.

Infectious Disease Director at Signature Health, Dr. Sarah Hoehnen, explains the importance of this initiative—“HIV is both treatable and preventable. Getting healthy and staying healthy all starts with a test. If a patient does not have HIV infection, medication is available that can dramatically reduce their risk of acquiring HIV. If a patient does have an HIV infection, treatment can help them live a long and healthy life,” said Dr. Hoehnen.

Program Details

• Free HIV and hepatitis C screening and testing

• The test is a quick finger stick with results in 20 minutes

• One does not have to be a patient, nor become a Signature Health patient

• Same- or next-day Infectious Disease appointments are available for individuals who test positive for HIV

• Connection to care is available for STI testing, medication to prevent HIV (PrEP), and hepatitis C

• A risk assessment screening will be completed prior to testing. Individuals at high risk for HIV will be offered a rapid HIV test. Connection to other resources will be offered for individuals at low risk.

Individuals at high risk for HIV should be tested at least annually for HIV, and the CDC recommends that every person be tested once their lifetime. See website for more details on HIV risks. For information regarding free HIV and hepatitis C testing at Signature Health: www.signaturefreetest.org, or call 440-578-8200.

I have purposely been outwardly silent, allowing and encouraging other voices to finally join this crucial conversation. Signature Health has been in the fight for what seems like forever. For too long the vast majority of the other members of our community have not focused on this struggle. Now that so many other people, organizations and leaders have begun to focus on systemic racism, stigma, and race related health disparities, and have joined us in this fight, I feel a sense of hope that together we have the opportunity to finally make meaningful progress on these issues. Since then there have been additional tragedies, including today’s article on Cleveland.com about a local African American man living with mental illness who was jailed for five months after being assaulted by Garfield Hts. police officers, that have brought all the anger and pain and struggle to a new crescendo, and I will no longer remain silent.

It has been so enraging that it takes repeated tragedy to get us all to finally see and to care. For systemic racism, it wasn’t until we saw yet another video of another person who was suffocated to death at the hands of a police officer. For the opiate epidemic, it wasn’t until thousands of young white kids started filling up the morgues after overdoses that we began to invest in treatment and started viewing addiction as a health issue and not a criminal one. How many babies have to die before we get serious about the infant mortality problem in our African American Community?

Let there be no doubt that Signature Health always has and always will fight for the rights and dignity of African Americans, and other stigmatized groups. We don’t say this now, nor do this, because it is the topic of the day. We have been in this fight from our very founding. It is a part of our core reason for existence.

We choose to take the enraging feelings and tragedy and turn them into resolve and action. We will continuously address the health disparities found in our community that lead to shorter lifespans and more chronic disease for our African American community members. We resolve to be a part of first responder teams to send mental health professionals to situations that are clearly needing our expertise. We will continue to be a part of educational programs to help lift people out of poverty. We will continuously focus on inclusion and stigma reduction with our staff, our patients, and our community, permanently. And we will continue to invite people to join us in this mission. Signature Health is a safe place for African Americans, people living with mental illness, and other stigmatized groups to work, to get the care they need, and to thrive and our resolve is to make the entire community just as safe.

Coronavirus Testing in Ashtabula

Ashtabula, OH (April 15, 2020)- Northeast Ohio-based Signature Health, a community agency providing primary medical care, infectious disease specialty care, mental health, and substance abuse services, is now offering COVID-19 testing at our Ashtabula location, 4726 Main Ave., Ashtabula, OH 44004.

Signature Health is now offering testing to our patients, Monday, Wednesday, and Friday between 9am and 11 am. The testing will occur in a designated area in the parking lot. If someone is not currently a patient of Signature Health, he or she can call our COVID-19 hotline at 440-578-8203 Monday-Friday, from 9am to 5pm to speak with a triage RN. “We’re doing everything within our power to offer resources for the Ashtabula community regarding COVID-19,” said Signature Health CEO, Jonathan Lee. “We chose our Ashtabula location as our first to offer coronavirus testing because there are fewer testing locations in Ashtabula County and transportation can be a barrier.”

Signature Health Chief Medical Officer, Dr. Libbie Stansifer, confirms that Signature Health continues to adapt quickly to serve our community. “Offering testing is the next logical step for us. Over the past several weeks, our leadership team has been consulting with local and federal health officials regarding COVID-19 and discussing anticipated community needs. We quickly established telemedicine services, pharmacy delivery and curbside pick-up, and a COVID-19 hotline with triage RN’s,” said Dr. Stansifer.

Dr. Sarah Hoehnen, Signature Health Director of Infectious Disease, adds, “We are eager to offer testing in Ashtabula. I think the important message to share right now is that we are here for the community and always will be.” Getting scheduled for a test is a simple. For patients already coming to Signature Health for primary care, the first step is talking with their doctor to determine if they should be tested. For people not currently coming to Signature Health, they can call the COVID-19 hotline 440-578-8203 and speak with a triage nurse.

Patients of Signature Health can learn more about COVID-19 by continually checking our website at www.signaturehealthinc.org/coronavirus.
Please contact us further with questions, concerns, or to speak to a Signature Health media representative.

Signature Health Providing Free Flu Vaccines for Children

Cleveland, OH (January 8, 2020)- Signature Health is now providing free vaccines for children, including the flu shot, through their Free Vaccines for Children program.

The Vaccines for Children (VFC) program is a federally-funded program overseen by the Centers for Disease Control and Prevention (CDC). Locally, the program funds are administered by the Ohio Department of Health.

Any child from birth through 18 years of age is eligible to receive a VFC-supplied vaccine if he/she meets at least one of the following criteria:

The child does not have health insurance.

The child is enrolled in Medicaid (including Medicaid HMOs).

The child is an American Indian or Alaskan Native.

The child has health insurance that does not pay for vaccine (applicable only to children attending a Federally Qualified Health Center (Signature Health), Rural Health Clinic or local health department).

The flu shot, along with other common vaccines, are offered free of charge to eligible children at all Signature Health locations across Northeast, Ohio. Adults may also get the flu shot (along with other necessary vaccines) at Signature Health. We accept Medicare and Medicaid and bill on a sliding scale fee for adult vaccines. Patients can call us at (440) 578-8200 to learn more, or schedule an appointment.

To avoid getting the flu, please refer to the below tips and quick informational video from our Director of Primary Care, Dr. Todd Wagner:

https://youtu.be/G0B4KBhhkww

Tips to avoid getting the flu:

Get the flu shot. Getting vaccinated is the single most important thing you can do to avoid getting the flu this season. We recommend patients get the flu shot starting in October, but it’s never “too late” to get the shot. Flu season starts in November, peaks in January and February, and cases have even been reported into the month of May.

Wash your hands thoroughly, and more often! The flu can be spread indirectly. For example, if someone coughs in their hand and opens the microwave door in your lunch room, and then you touch the microwave door, you could be at risk. It’s especially important to wash your hands before or after eating, after coughing (if you can remember, it’s best to not cough directly into your hands), and of course after using the restroom.

Minimize touching your face. Germs can easily spread by touching your eyes, nose, or mouth- and most of us do this without even realizing it. Try and be extra conscious to avoid this area, especially during flu season.

Disinfect common surfaces. Viruses that cause the cold and flu can live on common surfaces such as phones, door knobs, countertops, computer keyboards, etc. for up to 72 hours.

If at all possible, stay home if you are feeling sick. It may be tempting to go into work and push through your symptoms, but this is also a great way to spread the flu to your co-workers.

Signature Health Awarded $300,000 Grant to Provide Dental Services in Ashtabula, Ohio

Cleveland, OH (Sept. 20, 2019) - Cleveland-based Signature Health, a community agency providing primary medical care, mental health, and substance abuse services, is thrilled to announce they have been selected as one of 300 national grant winners by the Health Resources Service Administration (HRSA) to provide dental services in under-serviced areas.

The $300,000 grant awarded to Signature Health will allow them to provide dental services in Ashtabula, OH, an area with a severe dental service shortage. As a Federally Qualified Health Center (FQHC) since 2016, Signature Health has been able to expand their services beyond mental and behavioral health to include primary care. The addition of dental services at the Ashtabula facility adds yet another level to care and enables each patient that walks through the doors at Signature Health to be treated comprehensively.

CEO of Signature Health, Jonathan Lee, explains the importance of this dental grant- “The grant we received from the HRSA to provide dental services in Ashtabula really speaks to our agency’s mission of putting good health within reach of all our community members by providing comprehensive health care services,” said Lee. “At the end of the day, our vision is a community without health disparities, and this grant takes us one step closer to achieving that.”

Signature Health accepts both Medicaid and Medicare. According to the American Dental Association, only 38% of dentists in the U.S. accept Medicaid or Medicare. Being able to offer dental services in the Ashtabula community, and accepting a variety of payment options, will create a life-changing opportunity for Signature Health patients. In addition, patients who do not have insurance are billed based on a sliding income scale and those who choose to pay in full at the time of their appointment receive a 50% discount.

Signature Health plans to begin using their grant dollars beginning January of 2020 and hopes to start offering dental services by this time next year.

Signature Health Inc. Now Offering Free HIV Testing in Cleveland-Area Communities

Cleveland, OH (July 22, 2019) - Cleveland-based Signature Health, a community agency providing primary medical care, mental health, and substance use services is now offering free HIV testing at Signature Health locations.

Available Monday-Friday, from the hours of 9am-4pm, Signature Health will host this free, confidential HIV testing on a walk-in basis at their Lakewood, Ashtabula, Beachwood, Maple Heights, Painesville, and Willoughby locations (see website for full testing schedule). No appointment is needed to receive a free HIV test. To keep things confidential, individuals desiring a test should simply ask for “free testing” at the check-in window.

If a patient does have HIV infection, treatment can help them live a long and healthy life,” said Dr. Hoehnen.

In addition to free HIV testing, Signature Health offers a variety of healthcare services including (but not limited to):

Counseling services (individual and family)

Addiction services (individual and group)

Assistance with transportation

Pharmacy on-site for convenience

On-site laboratory for quick results

Signature Health also offers assistance regarding insurance and finances so that each person seeking medical services can get the help they desire. These services include financial counseling, assistance in applying for Medicare/Medicaid, and providing discounted rates based on income (sliding scale).
