Medication Assisted Treatment (MAT)

Opiate addiction is a chronic, progressive, and often fatal disease that is reaching epidemic proportions in our communities. The lack of coordinated, accessible services specific to opiate addiction is a huge barrier to recovery.

Signature Health makes recovery from opiate addiction accessible and attainable through our innovative Medicated Assisted Treatment (MAT) Program.

Medication Assisted Treatment (MAT) is a treatment method for long-term opiate addiction. The MAT Program uses either Suboxone or Vivitrol. Suboxone is administered orally (daily); Vivitrol is an injectable medication administered monthly. Use of Suboxone or Vivitrol in combination with comprehensive treatment services allows individuals to function effectively without intoxication, impairment or fear of withdrawal symptoms. MAT is provided conjunctively with a full array of outpatient and/or residential services. Treatment is collaborative and individualized. Involvement of family or other support persons is essential to achieving a successful outcome.  

The goal is to reach and maintain the best possible level of functioning while working toward a drug and alcohol free lifestyle.

Medication Assisted Treatment Process:

Week 1: Evaluation
After the client's initial phone call, a medical evaluation, CD assessment and psychiatric evaluation will be scheduled. The client will be assigned to a treatment team consisting of a CD case manager, a therapist and a physician. The date of induction will be scheduled.

Week 2: Induction
All inductions will occur on Mondays. During the week of induction the client will be at Signature Health from 9:00am - 3:00pm, Monday through Friday. From 9:00am - 12:00pm, the client will be monitored for medication tolerance before attending our Intensive Outpatient Program (IOP) from 12:00pm - 3:00pm. Suboxone will be picked up daily and the client will be required to complete 3-5 urine screens throughout the week.

Week 3-16: Stabilization
During this phase of treatment, the client will attend either daytime (12:00pm - 3:00pm) or evening (6:00pm - 9:00pm) IOP sessions 3 days a week. In addition to IOP, the client will meet with their physician once a week (or bi-weekly, as determined by treatment team). Nurse assessments, urine screens and medication counts will be completed weekly. The client will also be required to attend at least 4 community based self-help groups weekly to help maintain their sobriety. Once stable, the client and treatment team may meet to determine if Suboxone can be picked up on a bi-weekly basis.

Week 17-52: Maintenance
In the final phase of treatment the client will attend a weekly aftercare program as well as continuing to attend 4 community based self-help groups every week. Medications will be picked up on a bi-weekly basis and the client will comply with monthly urine screens. Additionally, the client and treatment team will collaborate to determine the level of continuing outpatient services necessary.

Admission Criteria:

  1. Must be at least 18 years old
  2. Diagnosis of opiate dependency
  3. Established residence within 50 miles of treatment facility
  4. Willing to participate in all treatment services
  5. Not allergic to Suboxone or Vivitrol
  6. Medically stable
  7. Stable on psychiatric medications
